Skilled Nursing Care
Provides skilled care to patients with acute, sub acute and chronic conditions, including post surgical care on orthopedics, neuro, cardiovascular, diabetes, respiratory, wound care. Provides patient education on medication management, wellness/disease management, patient safety, physician instructions and recommendations/precautions.
Physical Therapy
Provides comprehensive evaluations, designing and implementing of a plan of care, according to existing conditions/limitations, patient’s participation to set goals and expectations. Frequency and duration of treatment will be set according to dr’s orders, patient’s diagnosis, conditions/needs, oriented to recover mobility and function and decreasing pain response.
Occupational Therapy
Provides comprehensive evaluations, designing and implementing a plan of care, focused on rehabilitation of activities of daily living, oriented to recover independence and safety on self care at home, facilitating reinsertion on community activities.
Speech Therapy
Provides comprehensive evaluations, designing and implementing of a plan of care focused on limitations of speech, cognitive and swallowing deficits, associated with strokes, brain injuries, parkinson’s disease and aging process.
Medical Social Service
Addax social workers provides assistance on information regarding community resources, financial aid, transportation assistance, forms and applications, patient /family counseling, long term planning. It is not easy to easy to make long term decisions without the proper information and professional guidance.
Home Health Aide
Professional home health aide who can help with your daily needs including bathing, dressing, medication reminders, light housekeeping, during your recovery process.
